Serial absolute lactate value < 4 versus relative 10% reduction as a predictor of mortality in severe sepsis and septic shock

چکیده
Introduction Serial lactate measurements provide a valuable tool to risk-stratify patients, determine the presence of ongoing tissue hypoperfusion, and potentially evaluate efficacy of therapeutic interventions. Prior studies have shown lactate clearance to be a valuable predictor of patient outcomes, however some have defined adequate change in lactate by a decrease from baseline by 10% whereas others considered an absolute reduction to lactate < 4mmol/L. We hypothesize that a serial absolute lactate < 4mmol/L after initial resuscitation will be as effective a predictor of future mortality as a change by 10%.

Early evaluation of severity in patients with severe sepsis: a comparison with “septic shock” — subgroup analysis of the Japanese Association for Acute Medicine Sepsis Registry (JAAM‐SR)
Aim The purpose of this subgroup analysis of a Japanese multicenter registry, the Japanese Association for Acute Medicine Sepsis Registry Advanced (JAAM-SR-Advanced), was to identify early outcome indicators for severe sepsis that are useful and more objective than "septic shock." Methods Among 624 patients with severe sepsis registered in JAAM-SR-Advanced, 554 with valid serum lactate data w...

Sepsis is the systemic response to severe infection in critically ill patients. Sepsis, septic syndrome & septic shock represent the increasingly severe stages of the same disease. Despite the remarkable improvements in outcome, sepsis & septic shock remain an important cause of morbidity & mortality in children. This is a retrospective study among the patients who were admitted in the pediatri...
متن کاملCell-free plasma DNA as a predictor of outcome in severe sepsis and septic shock.
BACKGROUND Increased concentrations of cell-free DNA have been found in plasma of septic and critically ill patients. We investigated the value of plasma DNA for the prediction of intensive care unit (ICU) and hospital mortality and its association with the degree of organ dysfunction and disease severity in patients with severe sepsis. METHODS We studied 255 patients with severe sepsis or se...
متن کاملLactate clearance for death prediction in severe sepsis or septic shock patients during the first 24 hours in Intensive Care Unit: an observational study
BACKGROUND This study was design to investigate the prognostic value for death at day-28 of lactate course and lactate clearance during the first 24 hours in Intensive Care Unit (ICU), after initial resuscitation. METHODS Prospective, observational study in one surgical ICU in a university hospital. Ninety-four patients hospitalized in the ICU for severe sepsis or septic shock were included. ...

Objective: This study examined changes in lipid profile in patients with severe sepsis and septic shock in comparison with non-sepsis patients admitted to the medical ICU and its association with mortality. Materials and Methods: Through sampling of internal ICU admitted patients in Seman Kostar hospital, patients were divided into two groups of sepsis (n=107) and non-septic (n=115)....
